Quality palliative care and end-of-life support is required for people of all ages across all aspects of care. In practice, health care professionals provide services for people at different stages of their respective conditions, depending on their specialty and practice setting. Palliative care is centred on the patient and their primary carers. Therefore, health care professionals are required to have the appropriate skills and knowledge needed to provide quality end-of-life care.

GPs and practice nurses are invited to attend this End of Life Care and Advance Care Planning Symposium which includes:
  • Introduction to the Qld Health End of Life Care Strategy – Working together as a sector
  • The role of the GP in End of Life Care and Supporting Education
  • Medicare Item Numbers and Tools applicable to Advance Care Planning 
  • PEPA Program – GP Experience
  • Launch of the Healthy GC End of Life Care web page
  • GC Health Specialist Palliative Care Services Handbook of Palliative Care Launch
 
This is an opportunity to come together to discover the new and emerging tools, supporting the role of the GP in providing best practice care to those patients navigating their individual palliative process.
 
A buffet dinner will be provided to all participants from  6pm.
